::  Diver Waiver
This waiver must be completed for each person in your group that will be coming aboard any of our charter boats. Non-divers only have to fill out the second section regarding boat travel.

::  Dive Charter Manifest (for US/CDN Customs)
A list of all persons in your group including their address and Passport #/Driver's License # to be used on our manifest. Required for all groups that will be diving any wrecks in US Waters. Information must be TYPED into spreadsheet and returned to Sea N' Sky Scuba. Complete instructions included in file.
